I recently ran across and issue that was not caught before and it has to do with the forum_answer table for the forums. The issue is that two different topics could have answers with the same a_id. In order to fix this issue, log into your phpmyadmin or whatever you do to administer your database, and set a_id to the primary_key and then set it ti AI which means Auto Increment. This should fix any future issues.
